California Consumer Privacy Act Disclosures

These disclosures (the “Disclosures”) supplement the Healthjump Privacy Policy (the “Privacy Policy”) and are required under the California Consumer Privacy Act (“CCPA”).  The Disclosures only apply to residents of California.

Categories of personal information collected 

As required by the CCPA, the following describes the personal information we collect:

  • identifiers such as your name, alias, address, phone numbers, or IP address;
  • personal information, such as a credit card number or other information which is not an “identifier” but can be associated with or relates to you;
  • age, gender, or other protected classifications (only if you have applied for employment through our website);
  • commercial information, such as purchase or use data;
  • Internet or other electronic network activity information,  such as your interaction with our Services ;
  • geolocation data, such as the location of your device or computer;
  • professional or employment-related information (only if you have applied for employment through our website);
  • education information (only if you have applied for employment through our website); and
  • inference data drawn from the above categories.

Categories of personal information disclosed for a business purpose 

We share the above categories of information (other than inference data) with third parties as set forth in the “Information Sharing and Disclosures” sections of our Privacy Policy.  Such disclosures have taken place over the last 12 months. 

We do not sell personal information

We do not and will not sell your personal information. 

Right to Know; Right to Request Access to or Deletion of Personal Information

Under the CCPA, you may have the right to (i) know what personal information we collect, use, disclose and/or sell about you, and (ii) request access to or deletion of your personal information. To submit a request for these, you can call 800.673.6617 or submit an email to admin@healthjump.com with “CCPA Request” in the subject line.  We will ask you to provide certain information to verify your identity and residence.  Once we have verified that information, we will respond to your request in accordance with the CCPA.  If we deny your request, we will explain why. 

Right to Non-Discrimination for Exercise of Rights

You may not be discriminated against because you exercise any of your rights under the CCPA.

Authorized Agent

You can designate an authorized agent to make a request on your behalf if (i) the authorized agent is a natural person or a business registered with the Secretary of State of California, and (ii) you sign a written declaration that you authorize the agent to act on your behalf.  If you are using an authorized agent, we must be provided with a copy of the written declaration certified by a California notary public. Submissions should follow the same procedures set forth above.
